At the Outreach Service, we provide practical and emotional support 24 hours a day, seven days a week to help people live as ordinary life as possible.
Our team, which is as culturally diverse as the people we serve, has indepth knowledge, skills and experience of supporting people with complex needs, challenging behaviour, autism, sensory deprivation, dementia and epilepsy.
We focus our support on the needs and wishes of the individual, promoting independence, choice and ultimately a person-centred approach.
As each person is unique, Outreach is a flexible service that is individually tailored.
We can offer support with daily living skills, benefits, budgeting, college and job applications, supported employment and many more opportunities that can maximise independence.
Our Outreach team can also help with activities such as shopping, social outings, trips to theatres, cafes, sensory and art experiences and visiting places of worship.
We operate core team support which allows trusting relationships to develop. Support is flexible and arranged on a week by week basis to ensure people receive what they want and when it is suitable to them.
We pride ourselves in taking the time to answer all questions so people are equipped to decide if Outreach is the service for them.
We discuss wants, needs and aspirations during an initial assessment. We use this information to make sure we know enough about the person who needs our support to provide a service that works for them.
We monitor our customers’ achievements to enable them to find new ways to maintain and develop their independence.
